| Report Options | Use this field if you want the report to contain only a summary, only detail, or both. |
| Create CSV File | Select this check box to create a CSV file in addition to the standard version of the report. |
| Create XML File | Select this check box to create and XML version of this report. |
| Include Cover Page | Select this check box to include a cover page with this report. |
| County Clerk | Type the name of the county clerk preparing the report. |
| E-mail Address | Type the e-mail address of the person preparing the report. |
| Fax | Type the fax number of the person preparing the report. |
| Jurisdiction NOT Exercised | Select the check box or check boxes to indicate the jurisdictions not exercised by this report. |
| Prepared By | Type the name of the person preparing this report. |
| Preparer’s Phone | Type the telephone number of the person preparing this report. |
| Final Run | Select the check box to indicate this is a final run of the selected report.
The final run is the version of the report that is sent to the reporting agency. After a final run of the report has been
made, the report cannot be run again for the selected Reporting Period.
