| Custodial Parent Type | From the drop-down list, select the extended connection type for the custodial party to be included in this report |
| Report Mode | From the drop-down list, select the report type to be generated: draft or final run. |
| Suppress Zeroed Out Charges | Select this check box to suppress charges that have been zeroed out. |
| Group by Case Number | Select this check box to group report data by case number. |
| County | Type the county for which this report is being generated. |
| Office | Type the office for which this report is being generated. |
| Prepared By | Type the name of the individual preparing this report. |
| Telephone Number | Type the telephone number of the office generating this report. |
