| Start Date | Enter the date when the disposition starts. |
| End Date | Enter the date when the disposition ends. |
| Type | Select a type from the drop-down list. Click the arrow to view a list of codes, or press F4 to find a code. |
| Location | Type the placement location. |
| Comment | Type notes or comments related to the record. Click  for additional space. Click  to check the spelling of the text entered.
