Type the term length in the Years, Months, and Days fields.

Select the status of the sentence and the date on which the status changed.

If the sentence is stayed, select the time at which the sentence no longer is stayed.

Select this check box if there is credit for time served on the sentence. Then, complete the Years, Months, and Days fields with the amount of time credited.

Select this check box if a conditional release is allowed on the sentence. Then, complete the Years, Months, and Days fields to indicate after what amount of time release is available.
