Complete the following fields to generate the Indiana QCSR.
Indiana QCSR Fields
Field Name Field Description
Last Run and Accepted This field is automatically populated with the date from the last time the report was generated and accepted.
Report History Click this link to view when the report was generated previously.
Locations Click Image to select the locations that contain the data for the activity.
Reporting Period From the first drop-down list, select to generate a monthly, quarterly, or yearly report type.
From the second drop-down list, select the month or quarter for which you want this report generated.
Finally, from the third drop-down list, select the year for which you want this report generated.
Report Options Use this field if you want the report to contain only a summary, only detail, or both.
Create CSV File Select this check box to save the report as a CSV file.
Presiding Judge From the drop-down list, select the presiding judge for the court for which you are generating this report.
Court Type Type the court type for which this report is being generated.
County Type the county for which this report is being generated.
Court ID # Type the court ID number of the court for which this report is being generated.
Name of Preparer Type the name of the individual preparing this report.
Preparer’s Telephone No. Type the phone number of the individual preparing this report.
Preparer’s E-mail Address Type the e-mail address of the individual preparing this report.
Date Prepared Enter the date when the report is being prepared.
Regularly Assigned Judicial Officer Click the Assigned Judicial Officer link to view and maintain the regularly assigned judicial officers.
Cases Awaiting Judgment In the Reporting Judge (2a) field, type the number of cases awaiting judgment that the reporting judge or court has under advisement.
In the Special Judge (2b) field, type the number of cases awaiting judgment that the reporting judge has under advisement where the reporting judge is serving as a special judge.
Certified Problem Solving Court Type the certified problem solving court type and the number of cases assigned to the court.
Alternate Judge Time These fields reflect service by other judicial officers that are not specifically assigned to the reporting court. The time for temporary judges, pro tems, senior judges, and transfer judges should be reported in days or fraction of days.

Explanation & Comments Type any additional comments or explanations related to the report. Click Image for additional space. Click Image to check the spelling of the text entered.
Final Run Select the check box to indicate this is a final run of the selected report.
The final run is the version of the report that is sent to the reporting agency. After a final run of the report has been made, the report cannot be run again for the selected Reporting Period.
