From the drop-down list, select the agency for confinement.

Select the option for the minimum confinement sentence. Your options are a specified term, Life, Death, or Indeterminate. If a term sentence is selected, type the minimum term amount in years, months, days, and hours in the corresponding fields.

Select the option for the maximum confinement sentence. Your options are a specified term, Life, Death, Indeterminate, or Not Applicable. If a term sentence is selected, type the maximum term amount in years, months, days, and hours in the corresponding fields.

Select this check box if the confinement time should be served concurrently with another sentence.

Select this check box if the confinement time should be served consecutively to another sentence.

Select this check box if the confinement time should be served concurrently with a prior sentence.

Select this check box if the confinement time should be served consecutively with a prior sentence.

Select this check box if the confinement sentence is to be served on weekends.
