Select this check box to include the date that the appeals record was generated.

Select this option to include the time that the appeals record was generated.

Placement |
Select a location from the drop-down list in this field that you want the selected page detail information to appear on each
page of the appeals record.

Font Size |
Enter the font size for the selected page details information.
